MD LTD is a unique membership program offering highly personalized medical care and amenities to patients on a 24/7 basis. Dr. Leslie A. Squires created the MD LTD concierge experience in San Francisco to fulfill his patients' needs for personal service and 24/7 physician access. Dr. Squires has joined a growing number of physicians who have chosen to provide a premium "concierge experience" to their patients who join the program. He offers a qualitative approach to medicine that encompasses individual healthcare needs, preventive services, and lifestyle planning. Individual Membership Services and Corporate Membership Services are available with the MD LTD Program.

Concierge medicine empowers the patient to establish a personal, one-on-one relationship with their physician, instead of relying on insurance-driven protocols. MD LTD patients enjoy customized medical care by augmenting existing coverage with concierge membership. The MD LTD Program offers the following:

Dr. Leslie A. Squires
Dr. Squires is board certified in Internal Medicine and has been practicing medicine in San Francisco for over ten years. He graduated from Iowa State University, summa cum laude, with a BS in both Zoology and Philosophy. He received his MD from the University of Iowa and completed his residency in Internal Medicine at Saint Mary's Hospital and Medical Center in San Francisco. He has also served as a Clinical Instructor for UCSF at San Francisco General Hospital.

Dr. Squires developed the MD LTD Program so that he could practice medicine in a way that is optimal for both doctors and patients. His vision is a return to the time-honored Doctor-Patient relationship. How is the MD LTD Comprehensive Annual Exam different from the annual physical provided by my insurance?
The MD LTD Comprehensive Annual Exam is a preventive examination that is significantly broader than the physical that may be covered by insurance. Your 90-minute Comprehensive Annual Exam is a cornerstone of the MD LTD Program. Dr. Squires conducts a cardiovascular assessment with an electrocardiogram and body fat analysis. An introductory visit with a certified nutritionist will be provided, if requested. This information is used to develop a strategy for weight management. Dr. Squires can also help you create an exercise plan that will help you meet your fitness goals.
Is the MD LTD Program covered by my health insurance?
Some insurance plans may reimburse a portion of the services and amenities that are provided to members of the MD LTD Program. MD LTD does not replace health insurance coverage nor are the preventive services provided by MD LTD covered by all health insurance plans. For laboratory tests, diagnostic studies, imaging services, hospitalizations, emergency room visits, or specialist services, patients are expected to maintain health insurance coverage. MD LTD will provide complimentary claim submission for services not covered by the membership program.
Is the MD LTD Program a substitute for insurance?
The MD LTD Program is not a substitute for health insurance. Subsequent visits are billed to the patient's insurance plan. Any outside laboratory work or other studies are also the responsibility of the insured.
Is the MD LTD membership fee tax deductible?
Joining the MD LTD Program may be tax deductible. The membership fee may be reimbursed through group health plans pursuant to Sections 105 and 106 of the Internal Revenue Code, cafeteria plans under Section 125 of the Internal Revenue Code, flexible spending accounts (FSAs), health reimbursement accounts (HRAs), and health savings accounts (HSAs). Please consult your tax advisor for specific guidelines regarding your individual situation.
What if I have a medical emergency?
The MD LTD Program is not a substitute for calling 911 in the event of a medical emergency. However, please notify Dr. Squires after the paramedics are dispatched so that he can coordinate your medical care with the local emergency medical team and make an emergency room visit if appropriate.
What if I need to see a specialist?
Membership in the MD LTD Program includes facilitated referrals to specialists. This means that we will contact other doctors for you, arrange your appointments, discuss your case in detail with the specialist, and follow up with you about your visit to the specialist. In addition, Dr. Squires will talk to you in depth about any procedures or diagnosis, and develop an appropriate personal action plan for you. If you should require hospitalization, Dr. Squires will check in with you daily and visit you frequently. He will optimize your care by working closely with hospital staff.
What if I have a pre-existing medical condition?
A pre-existing medical condition will not exclude you from joining the MD LTD Program.
What if I am diagnosed with a long-term illness?
Dr. Squires serves as your healthcare advocate by consulting specialists if necessary, as well as personally researching your condition. We assist you in locating expert treatment options, and provide guidance for utilizing the existing healthcare system for your needs.
Are Medicare recipients eligible for membership in the MD LTD Program?
Yes. Medicare recipients are eligible for membership, subject to current Medicare regulations, and membership benefits apply only to those services that are not already within the scope of Medicare provisions. Please contact our office for further details.
Are HMO recipients eligible for membership in the MD LTD Program?
Unfortunately, state regulations prevent us from offering membership in the MD LTD Program to HMO recipients.
What is the membership fee to join the MD LTD Program?
Membership in the MD LTD Program is limited in order to ensure personal attention. The annual membership fee is determined on an individual basis. Membership payment options are flexible, depending on individual needs. You may be eligible to start your membership at a discounted rate, please inquire during normal business hours for more information. Additionally, the activation of a membership for your spouse or domestic partner will be given at a reduced rate.
